I'm testing fsvs 1.1.16 on a Debian system, using the official Debian package.

The USAGE file and man page mention a "Format of URLs" chapter, but this chapter
does not exist anywhere in the installed documentation. I only found it in the
"src/url.c" by grepping the source package.
